Key Responsibilities
Personal Care & Daily Living Support
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility
- Support safe transfers using assistive devices such as Hoyer lifts and wheelchairs
- Prepare nutritious meals based on dietary needs and preferences
Health Monitoring & Medication
- Monitor vital signs and observe for changes in health or behavior
- Administer medications according to care plans and privacy regulations
- Maintain accurate documentation of care provided and any incidents
Social & Emotional Support
- Encourage participation in community activities, outings, and recreational programs
- Provide companionship and emotional support
- Communicate effectively with families and healthcare teams
Requirements
- Previous experience in home health, assisted living, nursing homes, or group homes
- Knowledge of dementia, Alzheimer’s disease, autism spectrum disorder, and developmental disabilities
- CPR and First Aid certification (preferred)
- HIPAA and medication administration training (preferred)
- Ability to safely lift and assist clients with mobility
- Strong observational and documentation skills
